Note: Please send us a copy of
the deposit slip thru fax or e-mail indicating your name and the
amount deposited so we can properly acknowledge the said payment.
- Upon completion of the requirements, you
will receive a confirmation call, SMS or email from the DLSAA Cards
Officer.
- Our DLSAA Card officer will
process your application and will send your DLSAACard within 5
working days (approximately 2 weeks for mailing addresses
outside the Philippines).
- Walk-in
applicants or those who have sent their accomplished application
form and other requirements through their authorized
representative will have their DLSAACard released within 10 to 15
minutes.
A copy of the list
of partner-establishments and schedule
of access to the swimming pool are also given for free
together with your DLSAACard. |
