- Proceed
to the DLSAA
Office located at Room 202 2/F St. La Salle Hall of DLSU-Manila.
- Present your DLSU school ID and current
Enrolment Assessment Form (EAF) to the DLSAACard Officer.
- Secure and accomplish the application
form completely (please PRINT all necessary entries).
- Submit the accomplished form to the DLSAACard
Officer for validation and assessment.
- Proceed to the Accounting Office to pay the card processing
fee.
- Return to the
DLSAA Office and present your Official Receipt to the
DLSAACards Officer who will process your DLSAA Alumni-in-Residence Card.
- Claim your DLSAA Alumni-in-Residence
Card and list of partner establishments offering lots
of discounts and privileges:
